We provide support services to the elderly or ill, who wish to remain in their own homes but require assistance, either because of illness or age, which family members are not able to provide immediately or on a regular basis
Liaising with family or client advocate to ensure that the client is safe and properly cared for in their own home
Checking that essential aspects of wellbeing and home care are in place and ensuring that homes are maintained, nutrition is adequate and shopping is done
Liaising with full time carers and household assistance
Providing emotional support to the ill and isolated
Managing medication and medical and other appointments
Providing emotional and physical palliative care assistance to people living with a terminal illness and supporting everyone affected by their diagnosis to achieve the best quality of life
COVID-19 Precautions
All standard precautions will be taken to minimise the risk of spread of COVID-19 and other infectious diseases – hands will be sanitised, as will items brought into the home, a mask will be worn by all parties, a distance of 2m will be maintained wherever possible and clean protective outerwear will be put on prior to entering homes.
Services
A care plan will be compiled on appointment, to suit specific client needs and preferences. This plan will guide the relationship and be appended to the contract, and will be amended as required in consultation with the client and/or family representative, to address the following:
Type of care required
Preferred days and time for visits
Designated representative for the client – client themselves or specific family member
Specific tasks to be addressed during care
Preferred method of communication (email or print) (phone calls will be used if there is a matter of concern to be discussed or urgent feedback is required)
Live-in caring can be provided, based on models used in the United Kingdom. Other options for care can include regular household to assess the physical, mental, and emotional status and needs of the client.
The assessments will monitor the following:
Physical and mental state of client - is a consultation with a doctor/ physio/ chiropodist/ audiologist/ occupational therapist or other specialist needed?
Medication – adequate supply and if being taken?
Equipment to facilitate comfort of the client
Socialising needs (as appropriate), including organising for relevant spiritual care
State of house – cleanliness and organisation (identify need for support)
State of nutrition – is there an adequate supply of food, is food being eaten or accumulating/spoiling, is meal preparation adequate?
What food and household supplies are required?
Other services include:
Doing shopping or taking the client shopping
Preparing meals
Organising for appointments – as required
Medical - doctor/physio/chiropodist/audiologist etc
Personal care – hair dresser, other
Social visits / activities
Taking the client to doctor and other appointments
Taking the client on an outing
Following up on relationship with carer, if appropriate
Liaising with relevant agency for live-in carer
Organising for equipment to support client comfort
Organising for additional care during periods of illness or convalescence
Assisting with palliative care
We will feedback to family every two weeks or more often if required If additional support is needed, this will be discussed with the client and/or family member to determine the additional needs and the way forward.
